This song was allegedly inspired by the debate at "Piers Morgan Uncensored" show dedicated to an "inappropriate" Sam Smith's outfit on tour when the singer appeared in underwear and fishnet tights. During the debate, Douglas Murray said that one Smith's performance wasn't original and remarked that "Madonna did this 40 years ago".

Madonna unveiled a small clip of this collaborative single in a TikTok video posted one week ahead of the song's release on the 9th of June, 2023. She had a black bodysuit on and added pair of black fishnet stockings.

Sam Smith teased fans with a "surprise" performance of this single at one of his concerts at Manchester AO Arena on the 24th of May, 2023. He posted a teaser image on Twitter with the caption "'Unholy' won't be the last song", but it didn't happen because the singer was forced to end the concert mid-show due to a vocal cord injury.
