This song was based on the novel "Johnny Got His Gun" by Dalton Trumbo about World War I. James Hetfield was inspired by that book that was given to him by his older half-brother David Hale.

Metallica made their first music video with this song. In the video a soldier in the hospital bed spells "Kill me" in Morse code, because he doesn't have arms and legs, and can't see or communicate in any other way after he was terribly wounded.

The scenes from the film "Johnny Got His Gun" (1971) are used in the music video for this song. Metallica eventually bought the rights to the film after they were required to pay royalty fees for showing this video every time.

This song is the most-played one from the album. The band has been playing it live since the album's release.
