The title of this song refers to a sexual act. That's why MTV censored the music video's title as "Track #1". Some video jockeys showed the real title with their hands in front of their noses.
Maynard Keenan explained the use of the words "stink" and "fist" as a reference to a friend of drummer Danny Carey who "isn't afraid of getting his hands dirty".
Every chorus of this song differ having at first "Finger deep" then "Knuckle deep" and then "Elbow deep... Shoulder deep". It represents the idea of how people tend to receive more and more pleasure until the extreme point because over-stimulation numbs their senses.
The viewers of TV channel Scuzz put the music video for this song on the number-six spot of the "Top 50 Music Videos Of All Time" list, and on the number one in the list of the "Top 10 Most F*cked Up Videos".
