There are several theories who was the prototype doctor for this song, but all of them say that such a doctor would supply celebrities with drugs. Paul McCartney once said that the song is about a doctor who could cure "everyone of everything with all these pills and tranquilizers, injections for this and that; he just kept New York high."
John Lennon said in a 1980 interview about this partly-autobiographical song, "Mainly about drugs and pills. It was about myself. I was the one that carried all the pills on tour."
One article entitled "Twisted Tales: The Beatles' Real-Life Dr. Robert Had the Feel-Good Cure for Celebs" (2009) suggested that Bob Dylan could be "Dr. Robert" because he introduced The Beatles to pot.
Robert Fraser (a gallery owner) could be the inspiration for this song. He was a friend of The Beatles and The Rolling Stones and was known for providing drugs for his wealthy clients.
Aldous Huxley wrote a book "Island" (1962) where the main character Dr. Robert MacPhail treated the people under his care, mentally and physically, by giving them a psychedelic drug. Aldous Huxley was featured on "Sgt. Pepper's Lonely Hearts Club Band" album cover, so this theory of Dr. Robert's origin seems to fit as well.
